HIPAA Notice of Privacy Practices
This notice describes how medical and mental health information about you may be used and disclosed and how you can get access to this information. Please review it carefully. We are committed to protecting the privacy of your protected health information (PHI), which includes information about your mental health,treatment, and payment for care.

We obtain your PHI directly from you (for example, during appointments, intake forms, phone calls, or messages) and may also receive it from other healthcare providers, health plans, or authorized parties involved in your care. We follow federal (HIPAA) and state laws to maintain the privacy and security of your PHI.

How We May Use and Disclose Your PHI
We may use and disclose your PHI without your written authorization for these purposes:
Treatment: To provide and coordinate your mental health care (e.g., consulting with other providers involved in your treatment).

If we handle substance use disorder (SUD) records under 42CFR Part 2 (if applicable), additional federal protections apply. We will comply with the more protective rules.

We will obtain your written authorization for any other uses or disclosures. You may revoke it in writing at any time (except for actionsalready taken).

Your Rights
You have the right to:
• Access and obtain a copy of your PHI (psychotherapy notes generally excluded).
• Request corrections to inaccurate or incomplete PHI.
• Receive an accounting of certain disclosures of your PHI.
• Request restrictions on certain uses or disclosures (we are not required to agree to all, but must honor some paid-out-of-pocket requests).
• Request confidential communications (e.g., by secure method or alternate address).
• Be notified of any breach of your unsecured PHI.
• Complain to us if you believe your privacy rights have been violated.

To exercise these rights or file a complaint with us, submit a written request or contact us at connect@meahmodern.com. We will respond as required by law.We will not retaliate against you for filing a complaint.

Our Duties
We are required by law to maintain the privacy of your PHI,follow the terms of this notice, and notify you of any breaches of unsecured PHI. We may change our practices and will post any revised notice on our website.
